Then probably lots of roots were exposed or ripped of, and the entire rootball wasn't placed back like it should, combined with the constant moving around did the trick...

For future reference, why don't they like being moved around? Is it the actual motion or the changing of environments?
Also, can this type of bonsai be kept inside all the time? The reason I wanted one in the first place was to improve the feng shui of my bedroom

Thats evolution at its finest: plants have evolved being absolutely still during their lives...( there is more to it than that of course but that sums it up)
No, it can't . There is not one tree in the entire world that lives naturally indoors. Basic plant physiology: fotosynthesis asks for sunlight. Trees die inside, it's as simple as that

Probably you wound up with a lot of airballs in the roots, where soil fell out and then weren't filled in when you put the soil on top. Usually repotting involves combing out the roots and then pushing soil around to fill in those holes.

Before getting another tree you might need to get a new cousin.

does a grow lamp not provide enough light? I had read about other people growing bonsai indoors with grow lights when I was researching it before I bought it

Many have tried, and even more failed.Grow lights are not good enough- I don't think anyone here uses them, and for a good reason
