Portulacaria Afra - Ever seen this?

I've kept this jade in the same spot with what I believe is plenty of light (actual sun and artificial), tried to keep the soil at differing levels of moisture, and followed recommended fertilization but this guy is just sick.
You can see in my attached pics that the bark has peeled away from the trunk. Has anyone ever seen this? Can I bring this guy back from death's door?

L:ooks like the soil is so wet, you have fungus growing on top. These plant thrive in hot sunny climate. I do not know where you are, and what weather you are having. But if it is winter and dark/cool, they should be very dry.

You keep the soil to wet. They require full sun to be healthy. I only give the 11 I've got water every second week in spring and summer. In winter I give it hardly water(once in 5 weeks).
Remember if you give them to much water they suck to much water up for storage and that is why the bark is peeling off. The bark just cannot keep all the moisture in.
