Trees do loose leafs for different reasons. Minimal required information in order to help you is: what species is it? What is your location and climate? How do you care for the tree? And please provide a photo as well.

Hi, it's a Chinese Elm. I live in England but the temperature in the house is normally 19-20?C (66-68?F). The tree was misted 1x per week, watered 2x per week, and feed every other week. I've just reread the bottle and realised it said 2x per week not every 2 week so I will start dosing that to hopefully help it recover? Here is a picture.

The amount of fertiliser is not the problem, keeping it in the house is, and it also sounds as if you are not watering enough. It looks very dry

First of all. The basic rule for fertilizing trees is only when a healthy tree is growing. Your tree is not healthy and it is not growing season. So stop fertilize now. You can resumé fertilize in spring, but only if you see healthy growth.
Dieing indoor elms is proboably the most common topic in this forum. So I made an extensive video about elm care. Check it out and resumé this thread if there are any remaining questions:

Hi thanks both for your help i have watered daily this week and tiny leaves have started growing clearly i was not watering enough, I'm going to cut back some of the very thin branches so it can concentrate its growth to the bigger branches. I also wondered if repotting might help ?

If it is struggling the best thing you can do is let it grow out of the problems. Don't cut anything off, because it has already put some effort into the growth on those limbs. Don't repot. That is a huge shock.
Put it outside. If you are Northern Hemisphere and it is getting cold, you might want to shelter from hard winds - if there are hard, cold winds there. Otherwise, don't let it dry out and wait until spring.

OP is in UK where the chinese elm is fully hardy.
The key here is waiting. If you listen to the advises allready given the tree is allready outdoors. There the tree do not drink much and what is needed will be provided by the uk authum rain. The tree will get dormant and you can forget about it until spring.
From the video that I asked you to sheck:
Repotting is done in late winter or early spring but only if the tree is healthy.
Pruning is done on healthy growth during the season. Your tree is not healthy and it is not growing season.
If you do everything right you may see the tree burst into growth come spring. That is when you can start think about fertilizer and pruning. Repotting have to wait until after a full season of healthy growth, earliest late winter 2023-2024.
