Dead ficus

I am not sure how but I believe my ficus has died. It slowly but surely lost all of it's leaves no matter what I tried. I kept it moderately watered, watering it about once a twice a week, it was in a room that has plenty of light, but it wasn't in direct bright light, so I am not sure what went wrong. After noticing the last two leaves beginning to die I tried to do an autopsy and see if something had happened to the roots, but nothing seems out of place to me so I really don't understand what went wrong. Anyone with any ideas about what happened? :dry:

How long has it been with you? Where in the world are you? Spring or authum now?
Water two times a week sounds low, but can be ok depending on soil and pot.

I am based on the east coast of the USA, where we are heading into the autumn. In terms of watering, I would often feel the soil and it was pretty consistently moist, but not damp. I would water when the soil felt dry about 1 or 2 cm down. If anything I was a bit concerned about over watering as the soil did seem to hold moisture. The temperature indoors was consistently around 78 F. It is sat on top of a pebble tray that I would keep water in to increase humidity as well.

Is it your first authum with this tree? Have outside temperature dropped and triggered heating system to start warming yet?

Yes it is my first autumn with it, I only got this plant in May. While it has cooled, that has one meant the cooling does not turn on, haven't had to turn on the heat yet. But I had read about this so I keep the vents in that area of the house closed such that there isn't a blowing, even though the temperature in maintained. But also it started dropping leaves over the past 6 weeks. And it just never stopped.
